Neely-O’Brien 6^th Grade Volleyball Team

 

Dear parents,

 

Your child has indicated to me an interest in signing up for our volleyball team.  This is not a real team that competes in tournaments or a league.  We just get together for a few practices and play a few other schools.  My only requirements for joining this team are this permission slip, make all five practices, and have a great attitude/work ethic.  Please fill out the permission slip below and return as soon as possible.  Keep this top portion so you will know our schedule.  Please feel free to call me if you have any questions.
